This generally caused by bad mapping.

Look for a weird shaped object and delete it.
- If you have created a brush with multiple cuts, or have reshaped a brush.
- Make sure you are mapping with "align-to-grid".
- Do not use vertice manipulation on brushes
- Learn to use Detail/Structural/Non-coliding - See here -

I know the mistake, I made a broken bridge near end and I made multiple cuts on the surfacing. I can see the tiny brushes but I can't select them because they're too small, barely visable too the naked eye. I click Misc>Find brush but I can't find the brushes ID, Radiant didn't even make a error log so I can't even click File>Error file too find the brushes ID. It's not really down to bad mapping, it's a general mistake which I will learn by it...I hope :)

Please help ^^

Create a brush that SURROUNDS the brushes that are a problem.
Then look for a button on the toolbar - "select inside" (or "select touching") and delete.
