Radiant problem! Help!

Hi! So i am having a problem with radiant! Every time when i make some primitive shape, like cylinder or end cap
i get some error and radiant quits! Or, when i make a shape, it is there, everything is fine, and then i save it,
exit the radiant and when i load it again, that shape is there, but invisible, or i get some error and radiant just quits!
I don't know if i am doing something wrong or is it just some bug on tools!

it's about COD4 mod tools!

Please help me to fix that!
